1: Health Compliance at Florida State University
Welcome! Please take the time to read this page
completely. It is the key to assuring that you will be able to
register for your classes when you arrive at Florida State University.
This tutorial will explain:
1. Submission of the Health History Form which
includes verification
of required immunizations
2. On line proof of health insurance
3. State of Florida and Board of Trustees regulations for international
students
4. The subsidy available for selected graduate students to assist
in paying for health insurance
5. Third Party billing – when someone other than the student
will be paying the insurance premium.
Failure to complete items 1 and 2 will result
in a stop on your registration. These stops are called soft stops.
They can be removed by the student’s positive action to
meet the requirement.
Please note that your immunization records will not automatically
be transferred from your previous institution nor will FSU admissions
forward them to the Health Compliance Office when you send your
transcript to FSU. You must initiate this process upon acceptance
to FSU.
